Building intake and compliance forms
Overview. IdentityCheck includes a drag-and-drop form builder for collecting consent, intake and due-diligence information from your clients. You can build a questionnaire from scratch, add conditional logic and file-upload questions, or install a ready-made starter form from the catalog and adjust it to suit your firm. Finished forms can be attached to a verification or published as a public link clients fill in themselves.
How do I build a client intake form?
The quickest way to start is with a template. Open the form builder, install a ready-made starter form from the catalog with one click, and adjust it to suit your firm. That gives you a sensible, compliance-minded structure to edit rather than a blank page.
If you would rather build from scratch, you can add questions by dragging them onto the page. You choose the question type (short text, multiple choice, date, file upload and more), write the label, and mark whether an answer is required. There is no coding involved, and you can reorder or remove questions at any time.
To keep forms short and easy for clients, add conditional logic. A question can be set to appear only when an earlier answer makes it relevant. For example, you might ask for trust deed details only after a client indicates they are acting on behalf of a trust, or ask for extra source-of-funds questions only when a transaction is above a certain size. The client only ever sees the questions that apply to them.
Multi-entity intake forms, which collect details for several related entities in one submission, are available on the Growth plan and above, not on Starter.
Once a form is ready you can attach it to a verification so it is collected alongside the identity check, or publish it as a public onboarding link that clients complete without logging in. Drafts auto-save in the client's browser and restore if they close the page and come back, so a half-finished form is not lost.
Can I collect documents and files?
Yes. Add a file-upload question wherever you need a client to attach something, such as a proof-of-address document, a trust deed or a signed authority. Clients can upload as they fill in the form, including on a public link. Supported file types are PDF, JPG and PNG.
When you review a submitted response, you can read it on the Response tab, save it as a PDF, or download all attached files together as a ZIP. The PDF matches the copy that is emailed on completion, so you always have a clean record for your file.
Where are the ready-made forms?
The form builder includes a catalog of ready-made starter forms covering common compliance needs: consent, intake, risk assessment, and enhanced due diligence. Install one with a single click and it drops into your account as an editable form. The enhanced due diligence starter, for example, comes pre-structured with guidance sections covering relevant parties, source of funds and wealth, adverse-media and open-source checks, senior approval, controls and record keeping.
Starter forms are a beginning, not a finished program. Review the wording, add or remove questions, and make sure the form reflects how your firm actually works and what your own AML/CTF program requires. The catalog is maintained and updated by StackGo over time, so it is worth checking back for new templates.
